Improving Outcomes for Youth Involved in Multiple Systems of Care: Part III

The Stoneleigh Foundation and Georgetown University’s Center for Youth Justice hosted a virtual convening featuring an update on the groundbreaking Multi-Systems Integration Pilot Program in Pennsylvania’s Delaware and Erie counties and a conversation with cross-system leaders on the program’s successes and challenges.

Systems of care have historically struggled to work in a collaborative and effective manner to meet the complex needs of youth who are involved in multiple public systems, such as child welfare, youth justice, behavioral health, and education. In Pennsylvania, an innovative new program was launched in 2023 to strengthen these efforts. The Multi-Systems Integration Pilot Program draws upon the lessons and best practices of two national-model initiatives: the Pennsylvania Care Partnership, a service delivery approach that builds partnerships to create a broad, integrated process for meeting families’ needs; and the Crossover Youth Practice Model, an organizational approach designed to support youth who are fluctuating between the child welfare and youth justice systems.

This webinar examined the benefits of multi-systems integration, including: more coordinated and collaborative cross-system work; improved information-sharing, and a better use of youth- and family-centered teaming approaches in case practice. Most importantly, the webinar highlighted the child welfare, youth justice, behavioral health, and education-related outcomes that resulted at both a process- and population-level in Delaware County.
